We had a few extra concerns with my father's dementia.  Here's what we looked at.

* Consistent assignment of staff to get to know the patients, includes turnover rate.

* Individualized care with that knowledge, including recognizing subtle changes.

* Administration of medications, and oversight of prescriptions.

* Increasing mobility. 

* Reducing pressure ulcers (bedsores).

* Dementia wing access and egress security.

* Quality food.

* Housekeeping.

* Activities, socializing opportunities within and without the facility.

* Transport to appointments and shopping.

* Homelike environment vs. cinder block sparse room.

I would be looking for mild weather, large amounts of activities (Not necessarily within the community itself but within the surrounding areas). Low maintenance, so if I want to travel I don't come home to additional work. Inexpensive but healthy places to eat nearby. Houses either not touching or having one outside wall and the other be overly sound proofed. Has a pool open 24/7 and Hot tub, and a workout area.

My mother-in-law has macular degenration so she cannot see well enough to drive. So she needs one with transportation available to do the basics like grocery shopping and other normal errands.  She would also need a condo or a place that had yard maintenance, and perferably housekeeping as well.
